Arrêt de Travail en Ligne: How Sick Leave Certificates Actually Work in France

Six-forty on a Thursday morning in Vénissieux. Karim has been awake since four, vomiting, and his shift on the warehouse floor starts at eight. He is not agonising over whether he is well enough to work — that question answered itself two hours ago. He is on his phone searching for an arrêt de travail en ligne rapide, because his team leader will want paperwork, and because he knows from a colleague that missing a deadline somewhere in this process costs money.
(Karim's situation, and the worked examples later in this article, are illustrative composites used to explain how the process works — not real patients.)

What Karim needs is not sympathy. It is an accurate map of a system that is genuinely fiddly: which document counts, who has to receive it and by when, why the first days are often unpaid, and what a doctor consulting him by video can legitimately produce. This article gives that map, including the parts that are inconvenient.
Need to speak to a doctor today? Choose an appointment time that suits you and a licensed physician joins at that exact slot — no queue, no waiting on hold. Book a consultation or see which consultation fits your situation.
The document that actually matters: l'avis d'arrêt de travail
French sick leave runs on one specific document, the avis d'arrêt de travail. It is not a letter, not a note on a compliments slip, and not a screenshot. It is a standardised form completed by a doctor, and it does two jobs at once: it tells your employer that a physician has judged you unfit to work for a defined period, and it opens the file at the Assurance Maladie that may pay you indemnités journalières, the daily allowance.
Historically the form came in three parts, the famous volets. Parts one and two went to your CPAM, part three to your employer. Increasingly the doctor transmits the medical portion straight to the Assurance Maladie electronically, which leaves you with only the employer copy to deal with. The Assurance Maladie has also tightened the paper version itself, moving to a secured form specifically to reduce forged certificates. The practical lesson is unglamorous: ask the doctor at the end of the consultation whether the transmission has been sent electronically or whether you are responsible for posting anything.
Two things this document is not. It is not a diagnosis for your employer — the medical reason stays between you, the doctor and the Assurance Maladie's medical service. And it is not retroactive on request. A doctor certifies from the point of assessment. Asking for a certificate covering last Tuesday, when nobody saw you last Tuesday, is asking for something a careful physician will decline.

Arrêt de travail en ligne: what "en ligne" actually refers to
The phrase gets used for two different things, and conflating them causes most of the confusion.
The first meaning is administrative. The form travels online instead of by post. Your regular GP sees you in the surgery, presses a button, and the CPAM receives the medical part within minutes. Nothing about the consultation was remote. This is now the normal way the document moves, and it is why fewer people handle three paper slips than a decade ago.
The second meaning is clinical: a doctor assesses you by video and issues the certificate from that consultation. This is possible, and it is also regulated more tightly than people expect. French law caps how long an initial sick-leave certificate issued purely by teleconsultation can run — three days — and a renewal cannot stretch the total beyond that limit either. The cap does not apply when the certificate comes from your own declared médecin traitant, or your referring midwife, or when you genuinely cannot reach a doctor for an in-person examination.
That rule exists for a reason worth respecting. Certification of unfitness is a medico-legal act with money attached. The legislator's judgement was that a short remote-issued absence is proportionate, while a longer one should involve either a doctor who knows you or an examination. Any service advertising a guaranteed two-week signature after a five-minute video call is describing something the law does not permit.

The deadlines, and what missing them costs
You have 48 hours. That is the number to memorise. Within forty-eight hours of the certificate being issued, your employer must receive their copy, and the Assurance Maladie must have the medical part — either through the doctor's electronic transmission or by your post.
Miss it and the consequence is not a rejection but a reduction. The CPAM can cut the allowance for the late period, and repeated lateness is treated less generously than a first slip. On the employment side, an unexplained absence is a disciplinary matter independent of your health, which is why telling your manager on the morning itself — a phone call, then the document — matters as much as the paperwork.
The 48-hour clock has cousins worth knowing. A prolongation, an extension of the absence, should normally come from the doctor who issued the original certificate or from your médecin traitant; extensions from an unrelated third doctor can be refused. And if you are unemployed, on a fixed-term contract that ends mid-absence, or working several jobs, notify your CPAM about the situation rather than assuming the standard path applies.
Le délai de carence: why the first days are usually unpaid
Here is the part that surprises people who have worked in other countries. For private-sector employees in France, the Assurance Maladie applies a waiting period — the délai de carence — of three days. The daily allowance starts from the fourth day of the absence. Public-sector staff have their own, shorter arrangement.
Now the nuance that changes the arithmetic completely. Many employees are protected by maintien de salaire: either their collective agreement or the statutory continuation-of-pay rules require the employer to keep paying, in whole or in part, often after a minimum length of service. Where that applies, the waiting period becomes invisible to you because your employer bridges it. Frequently the employer also uses subrogation, receiving the allowance directly and paying you as usual, so you never see a separate CPAM payment at all.
Which is why the honest answer to "how much will I lose?" is: look at your convention collective before you look at anything else. Two people with identical illness, identical certificates and identical salaries can end the month hundreds of euros apart purely on the strength of their sector agreement and seniority.

Which document fits which situation
The following scenarios are illustrative, not accounts of real patients.
| Document | Who issues it | What it does | Who receives it | Key limit |
|---|---|---|---|---|
| Avis d'arrêt de travail | A doctor working within the French system | Certifies unfitness and opens the daily-allowance file | Employer plus CPAM within 48 hours | Three-day cap when issued purely by teleconsultation outside your own care pathway |
| Private medical certificate | Any licensed physician, including remotely | Documents an assessment, findings and advice on fitness | Employer, school, insurer, or your own file | Does not trigger French daily allowances by itself |
| Certificat pour garde d'enfant malade | Doctor assessing the child | Justifies a parent's absence to care for a sick child | Employer | Paid or unpaid depending on your agreement |
| Hospital discharge summary | Hospital team | Evidences admission and recommended recovery time | Employer and CPAM | Still needs a proper sick-leave form for allowances |
| UK fit note | UK clinician | Evidences unfitness to a UK employer after self-certified days | UK employer | Not a French document and not interchangeable |
| Insurance or travel medical report | Any licensed physician | Supports a claim, a refund or a change of travel dates | Insurer or airline | Each insurer sets its own evidence requirements |
Read the table row by row against your own situation rather than looking for the best row overall. Karim needs the first one. A freelancer cancelling a trip needs the last one. Confusing the two is how people end up with a document nobody wants.
Two worked examples
The examples below are illustrative composites rather than real patients.
Karim, the warehouse shift. He calls his team leader at seven and says he cannot come in. He then consults a doctor the same morning. If that doctor is inside the French system and assesses him as unfit, the certificate is issued, transmitted electronically, and Karim sends his copy to the employer immediately. Because his collective agreement continues his pay after a qualifying period of service, the three-day waiting period is absorbed. Total damage: one lost day of overtime, no lost allowance. What made it work was the phone call at seven, not the paperwork at eleven.
Élise, the freelance translator in Nantes. She is auto-entrepreneur, which changes the picture entirely. Self-employed workers have their own contribution-based conditions for sickness allowances, with different qualifying rules and waiting periods, and they answer to no employer. What Élise actually needs is a clear written medical assessment to justify postponing a delivery to a client and, if applicable, to support a claim under her own insurance. A private consultation is well suited to that. The statutory employee machinery is simply not the framework she is in. Working out which framework applies to you before you request a document saves a great deal of frustration — the frequently asked questions page covers what kind of written documentation a consultation can and cannot produce.
Your obligations while signed off
A sick-leave certificate comes with duties, and people who are unaware of them get caught. You are expected to remain available at the address the CPAM holds, respecting any authorised outing hours written on the form. Leaving your département or travelling abroad during the absence generally needs prior authorisation from the CPAM. Medical control visits happen: the Assurance Maladie's medical service can call you in or send a practitioner, and your employer is entitled to arrange a contre-visite by a doctor of their choosing.
None of that is designed to punish genuinely ill people. It exists because the allowance is public money. Practically, tell the CPAM if your address changes during the absence, keep the form where you can find it, and do not treat a signed-off week as free holiday time — the one behaviour that reliably turns an administrative process into a dispute.
What a private online doctor can and cannot issue
This is where honesty matters more than marketing. A licensed physician consulting you remotely can assess your symptoms properly, explain what is happening, advise on treatment, prescribe many routine medicines electronically, and produce a written medical certificate describing the assessment and their opinion on your fitness to work. Many employers accept such a certificate for a short absence, particularly for international staff, and insurers frequently prefer a clear clinical letter to a bare statutory form.
What such a consultation cannot do is manufacture French statutory entitlements. It cannot guarantee indemnités journalières. It cannot exceed the legal limits on remote certification. It cannot backdate. It cannot examine your abdomen, listen to your chest, take your blood pressure, order same-day bloods, or read an X-ray that does not exist. It does not prescribe controlled medicines. And it is the wrong place entirely for an emergency.
Used within those boundaries, remote consultation is genuinely practical. You choose a time in the calendar and the doctor joins at that time, so you are not sitting in a waiting room while feverish, or holding a phone line open hoping to be reached. Consultations begin from €39 for a standard consultation, with sick notes and prescription renewals from €35, and they are carried out by licensed and registered physicians — the clinicians available for appointments and the range of services offered, including sick notes and medical certificates, are both published openly. If you want to understand the model before booking, the description of how the service operates is the place to start, and appointments can be arranged directly from the main consultation platform.
Red flags: when paperwork is not the priority
Some symptoms mean you should stop thinking about your employer entirely and call for help. In France, dial 15 for the SAMU or 112 anywhere in the European Union if you have chest pain or tightness lasting more than a few minutes, sudden breathlessness, one-sided weakness or facial droop, sudden difficulty speaking, a first seizure, uncontrolled bleeding, a rigid painful abdomen, repeated vomiting with severe dehydration or blood, fever with a stiff neck and a non-fading rash, a sudden worst-ever headache, or thoughts of ending your life. 18 reaches the fire service, 114 serves deaf and hard-of-hearing callers by text, and 3237 tells you which pharmacy is open.
Hospital care produces its own documentation. Nobody in an emergency department will refuse to evidence your absence because you arrived without a form.
Frequently asked questions
Peut-on obtenir un arrêt de travail en téléconsultation ? Yes, within limits. A doctor consulting you remotely may issue a sick-leave certificate, but the law caps an initial remote-issued absence at three days, and a renewal cannot push the total beyond that. The cap lifts when the certificate comes from your own médecin traitant or referring midwife, or when an in-person examination is genuinely out of reach.
Comment obtenir un arrêt de travail rapidement le matin ? Call your employer first, then secure a same-day consultation by whichever route is quickest for you: your own practice, the Service d'Accès aux Soins through 15 for urgent needs, or a private appointment in a slot you pick. Then handle the 48-hour transmission the same day rather than at the weekend.
Combien de jours de carence pour un arrêt de travail ? The Assurance Maladie applies a three-day waiting period for private-sector employees, with allowances starting from the fourth day. Public-sector staff have a shorter arrangement. Whether you actually lose income depends on your collective agreement, since many employers continue pay over that period.
Est-ce que mon employeur connaît la raison de l'arrêt ? No. The medical reason goes to the Assurance Maladie's medical service, not to your employer. Their copy shows the dates and the fact of certified unfitness. You are not obliged to disclose a diagnosis to a manager.
What if I fall ill in France but work for a foreign employer? Then the French statutory form may not be the document you need at all. A UK employer expects a fit note after the self-certification period, an Italian employer works through the INPS certificate, and a Spanish employer through the baja médica. In cross-border situations, a clear written medical assessment plus a conversation with your HR department usually resolves it faster than chasing a French form. Background reading, if you want it: Online Doctor Consultation — Online Doctor Consultation.com, Online Doctor Consultation.es, Online Doctor Consultation.eu — publishes general health information, European resource pages look at other European systems, and Spanish-language material covers the same ground in Spanish. None of them issue prescriptions.
Can a doctor extend my sick leave online? An extension normally has to come from the doctor who issued the original certificate or from your declared referring doctor, and the same remote-certification limits apply. If your absence is going to run long, that is a signal to get in front of a doctor in person rather than to keep renewing remotely.
My employer refuses to accept a private medical certificate. What then? They may be within their rights, because the statutory form is what French employment and social security processes are built around. Ask specifically which document they require, in writing, then obtain that one.
A short checklist for the morning it happens
Tell your employer before your shift starts, by whatever means your contract specifies. Get assessed the same day. Ask the doctor whether the medical part has been transmitted electronically or whether you are posting it. Send the employer copy immediately, keeping proof of sending. Check your collective agreement for continuation of pay and the waiting period. And note the end date, because a return without a completed absence is as awkward as a late certificate. If a specific detail about documentation is unclear before you book anything, ask through the contact page rather than guessing.

It is not medical advice, legal advice, or a statement of your entitlements, and the rules governing sick leave and remote certification change. Confirm current requirements with the Assurance Maladie or your employer, consult a qualified clinician about your own condition, and contact emergency services immediately if you are seriously unwell.
